Apparently this American Academy of Pediatrics - AAP form has been out for a couple of years - see link below. Note also from above screenshot the last sentence referring to "...immediate medical evaluation and tests that might not be necessary if my child had been vaccinated". Looks like an insurance-voiding clause if ever there was one - Your child was not vaccinated...well in that case the insurance will not cover the treatment....etc.. For reference:

The "Refusal to Vaccinate" form was created by the American Academy of Pediatric's 'legal department' as a response to the growing number of toxic vaccines recommended by them and the growing number of parents who are becoming educated on this issue.
